You can choose to either book your appointment online or register on the waitlist if the doctor you are seeking is not currently accepting new patients. Alternatively, you can visit the clinic in person and our lovely staff will help you
In case of a medical emergency you should call 911 or go to your nearest Emergency Room. You can view the ER wait times here.
If you would like access to your medical records, please contact our office and submit a request in writing and sign the consent to release your medical records. Please be aware that there is a fee for this service as per the Health Information Act.
- The following uninsured services are NOT covered by the AHCIP:
- Transfer of medical records
- Telephone prescription renewals
- Travel advice and immunizations
- Some de-insured procedures (e.g., circumcision, wart removal)
- Forms, reports or letters*
- *You or a third party – such as an employer, insurance company, lawyer or school – may request your physician to complete a form, report, letter, etc. Provision of these types of uninsured services requires the time, expertise (and office materials) of your doctor and her/his staff. In some cases, a physical examination may also be required. You (or the third party) will be charged a fee to cover these costs.
